By submitting or uploading your video or other material, you give consent to Americans for Healthy Communities (AHC) to use and publish the information you provide,  and to use and publish your name, voice, photograph, video image, and/or any statements you have made in print, on the Internet, other TV and radio, or through any other media worldwide in perpetuity. By submitting or uploading any video or other content, you waive any right that you may have to inspect or approve any finished products incorporating this material, and you agree to release AHC from all liability and any claims you may have arising out of the use, publication, or disclosure of this material. To the extent any of these materials are copyrightable, you further grant permission for AHC to copyright this material, and you release AHC from any liability arising from any blurring, distortion, alteration, optical illusion, or composition of your photograph or video image. You acknowledge and agree that AHC shall have sole ownership rights to any images and video of you and comments made by you which are contained in any printed material, videos, digital and online media, advertisements, and any other material, and hereby relinquish and waive any ownership interest you may have in such material, to the extent that any such interest exists. You further acknowledge and agree that AHC, as the owners of any images and video of me and comments made by me, shall have the right, in their discretion, to edit any images and video of you and comments made by you which are contained in any printed material, videos, digital and online media, advertisements, and any other material without payment to you. You further acknowledge that AHC may freely assign any and all of their rights described in this Release to other entities allied with AHC in the effort to educate the public on the U.S. health care system and to promote healthy communities and access to adequate fact- and science-based medical care.

By submitting the video or other materials, you are representing to AHC that you are at least 18 years of age and have every right to grant these permissions and releases in your own name, and you  confirm that all information in the material that you have or will provide to AHC is true and accurate, to the best of your knowledge.
